Questionnaire

The Questionnaire Module can be added during the sourcing event creation process from the Event Builder.

Adding a Questionnaire

  1. In the Event Builder, use the “Add Module” button to add the Questionnaire.

  2. If your event template includes a questionnaire, it will appear by default.

Once added, the questionnaire becomes part of your event structure.


Building Your Questionnaire

You have multiple ways to populate and structure your questionnaire:

1. Generate Questions with KIT

  • Use the Ask KIT button to automatically generate sections and questions.

  • KIT pulls context from:

    • Event content

    • Intake request

  • KIT returns suggested sections and questions which can be reviewed and edited.

Tip: KIT can also help enhance questions to improve your existing questions.


2. Manually Add Sections and Questions

  • Add Sections

    • You can add sections in the Event Builder or full-screen questionnaire view

  • Add Questions using:

    • Top-right Add Question button

    • Bottom-left Add Question shortcut

    • Inline entry within each section

Each question includes configurable fields:

  • Question Text

  • Question Type (e.g., short answer, date, dropdown)

  • Required checkbox

Supported Question Types

  • Short Answer

  • Paragraph

  • Numeric

  • Currency

  • Date / Date Range

  • Phone Number

  • Email Address

  • Rating Scale

  • Dropdown (Single Select)

  • Multi-select Checkbox

  • Yes/No Toggle

  • Attachment

  • Percent

  • Conditional Logic


3. Import Questions from Excel

If you prefer working offline or reusing past content:

  1. Export the Questionnaire CSV template.

  2. Populate your questions and sections in Excel.

  3. Re-upload the file using the Import option in the questionnaire builder.

  4. Fairmarkit will parse the content and rebuild your questionnaire structure.

Tip! This feature is great for bulk uploads and reusing previous events.


4. Navigation & Layout

  • Sections appear in the left panel and can be renamed or reordered.

  • Use drag & drop to rearrange questions or sections.

  • You can switch to full-screen mode for easier navigation and editing.
